The card is divided into a number of fields for recording information about orders and receipts. Each field is divided by a solid line running from rows 1 to 9, with the field titles replacing row 1. A full list of fields in the field legend is outlined in the appendix below.
In the centre of the card is a legend “Receipts - Green Stripe” and “Rec. Canc. - Brown Stripe” indicating these cards were likely reused for both computerised order entry and receipt entry. The card is printed with “716072”, “General Railway Signal Co”, “282” and “EDM15152” in the bottom margin. All columns are numbered below the 0 row, and below the 9 row.
