The card has a number of fixed length fields separated in the punch area by solid lines with the field labels printed between rows 4 and 5. The Quantity, Standard Price and Extended Amount fields provide space for decimal values by separating the last 2, 3 and 2 digits of each field respectively with a dashed line. The print area at the top of the card repeats the field titles in a condensed format so that fields can be easily identified if print output is provided on the card.
The card has “Rocky Mountain Arsenal” printed the right margin and “SARRM FORM L-40” and “(18 MAR 1974)” printed in the left margin. “Globe 508991” is printed in the bottom margin. The card has column numbers for every column, below the 0 and 9 rows.
