Cream 80-column punch card printed for the State of Illinois, with square corners and a card cut on the top right-hand corner. The card has a printed orange band at the top for easier identification. The card is a form for “STATE OF ILLINOIS, VOCATIONAL EDUCATION, REIMBURSEMENT STATISTICS,” intended for reporting expenditures and reimbursements statistics related to Illinois State’s vocational education system.
The print area carries a multi-line header that divides the print area into fields corresponding to the fields punched into the punch area. These fields are outlined in the appendix below. Fields are seperated by vertical solid lines, with some fields providing additional horizontal subdivision with dashed lines indicating a thousand separator in currency fields, and a decimal point in percentage fields.
The punch area is overprinted with repeated row numerals “0” through “9” on their respective rows. The boxed label “SCHOOL BOARD NAME AND ADDRESS” is printed between columns 55-75 under row 4. The boxed label “PROGRAM, PURPOSE OR TYPE EXPENDITURE” is printed between columns 55-75 under row 4. Some fields are labeled with an alphabetic code below row 7, such as “A”, “B”, “C”, etc. Several fields are also labelled with a algebraic expression (eg. “B-E”) below row 8, which indicates how the field is calculated from the other labelled fields.
All columns are numbered from 1 to 80 underneath the 0 and 9 rows. The bottom margin reads “644268 BSC” at left and “STATE OF ILLINOIS, VOCATIONAL EDUCATION, REIMBURSEMENT STATISTICS” centered. The reverse side is unprinted.
