Cream 80-column punch card printed for use by a “Credit Underwriting Department”, with square corners and no card cut. The card serves as a control/balance card code sheet for premiums and is is intended to be filled out by hand and then punched into a detail card. This card was never intended to be punched and likely would have had a corresponding blank detail card for data entry based on the fields and positions in the appendix below.
A title in the top margin reads “CREDIT UNDERWRITING DEPARTMENT CONTROL/BALANCE CARD CODE SHEET”. below the title is a table of fields and their punching position on the card, along with blank space for the fields to be filled out. At the bottom of the card are several additional fields to aid the user in calculating totals for the premiums. There are also instructions for the user regarding the use of several of the fields (noted in the appendix below).
Production code “DD-ZD 10232” is printed in the bottom margin. The reverse side is blank.
