Cream 80-column punch card printed for Bell Telephone Laboratories, with rounded corners and a card cut on the top right-hand corner. The card features a screened Bell System emblem centered in the punch area this is an older version of the Bell logo used prior to 1969.

The left margin reads “GENERAL APPLICATIONS CARD.” The right margin carries “E-7583-E (7-66)”. Along the bottom margin are “BELL TELEPHONE LABORATORIES, INCORPORATED” and “ISC 38002.” Within the punch area, each row is printed with repeating numerals “0” through “9” corresponding to their respective rows. All columns are numbered from 1 to 80 above the 0 row and again below the 9 row. The reverse side is unprinted.
