Lewis Business Forms
Lewis Business Forms was based in Jacksonville, Florida, United States of America and produced a number of paper products for business including punch cards. The company was established in 1946 by M. G. Lewis[1]. It registered subsidiary companies in 1958 specialising in tabulation card printing[2] and appears to have print facilities in both Georgia and Florida by the 1960s.[3] A photograph from 1963 describes Lewis Business Forms as “one of the largest printing companies in Florida”[4].
